Use this form to make an on-line enquiry regarding an event and reserve the date if it is available.

When you click on 'submit' an email is sent to Sixth Moon Art and the details will be saved.

Sixth Moon Art will then send you a quote. When an agreement has taken place and the deposit has been paid, the contents of this booking record, with the quote added, will become a contract of agreement.

Make a note of the reference number assigned to this booking (it will also appear on your quote), as you will need it to pay your deposit on-line. Alternatively, for an initial no commitment enquiry, use the Contact page.

Notes on the Form

Asterixed fields are mandatory.

The Age field is mandatory for children's events.

The time range (From Time / To Time) should be the time for which the artist is needed and on which the quote will be based - the artist will arrive 15 minutes before this time to set up and will stay a further 15 minutes after to pack up.

If the event will take place at the contact address, use the checkbox to copy the information across.
